If you want to do your own, best bet would be to work directly on top of the original one. It's a lot of work, but there's not template that i'm aware of. Most people seem to make them from scratch or animate over the original opening. If you look at the Dark Souls one, you'll notice that the backgrounds aren't exactly the same.

Short version, I took that initial bit from evangelion (that burst) then googled image searched for a "nebula" that looked as close to the one in the evangelion opening and changed the color of it in photoshop. You could probably chromakey out a lot of the unwanted elements from the original intro to leave you with the 'background elements'.
